Patient Information - What to Bring

We ask that you bring:

1. Driver's license or other photo ID
2. Current health insurance card and
3. Utility bills or other correspondence showing current residence if the photo ID does not show the patient's current address. If the patient is a minor, the patient's parent or guardian should bring the information listed above.
4. A referral if required by your insurance (HMO)
5. A current list of your medications, their dosage, and how often they are taken.
6. Your current eyeglasses and/or, if available, a current copy of your contact lens prescription or lid top.

For neuro-ophthalmology patients: Please bring copies of prior imaging studies and any relevant medical records.

It is our expectation that you will pay your copay and/or visit charges as required by your insurance at the time of your appointment. We do accept Visa, MasterCard, and Discover for your convenience.

There is a possibility that you may need to have special testing and/or procedures done which can cause your visit to last one to three hours. We make every attempt to see patients at their scheduled times, but unforeseen emergencies may cause a delay.

  This allows us to share the results of your testing with your referring doctor, your primary care doctor, or anyone else you wish.

• 3-page History Form
  This helps our doctors better diagnose and manage your condition.
• Authorization for benefit assignment
  This allows us to report information to your insurance carrier so they will pay for your visit.
• Request for medical records
  This form allows us to obtain records from your other doctors and share your exam findings with them as well.
